What is a dental implant?
A dental implant is a small titanium post placed into the jawbone to act as an artificial tooth root. Once the bone has healed around it, a crown, bridge or denture is attached to it.
That's the important difference from every other way of replacing a tooth. A bridge leans on the teeth either side. A denture rests on the gum. An implant is anchored in the bone, which is why it doesn't move, doesn't click, and doesn't need the neighbouring teeth to be cut down to support it.
It also does something no other option does: because the post is in the bone, it keeps loading that bone when you chew. Bone that isn't loaded shrinks away — which is why long-standing gaps change the shape of the face over time.
The post sits in the jawbone like a natural root. That's why an implant doesn't move when you eat — and why it keeps loading the bone that a gap would let shrink.

Dr Tayebeh Mobaleghi carries out implant treatment here. She earned her DDS in 2009 at SB University, Iran's top-ranked medical science institution, followed by a clinical MSc in Periodontics between 2013 and 2016, and later advanced study in oral pathology at Queen Mary University in London. That combination matters more than it might sound. Periodontics is the study of the gum and bone that hold teeth in place — which is precisely what an implant is placed into, and precisely what determines whether it lasts.

There is more than one way to replace a missing tooth, and an implant isn't automatically the right answer. Here is the honest comparison.

Implant-retained bridge

Implant-retained dentures
For anyone whose denture has never sat properly. The denture clips onto multiple implants, so it stays put when you eat and talk — a much more secure fit and a stronger bite than a denture resting on the gum.
Teeth-in-a-Day
A full row of teeth restored in a single day: four to six implant posts are placed and a connected bridge is fitted to them straight away, so you don't go home without teeth. It can often be done even where bone has already been lost.

Step by step
How long does getting an implant take?
Consultation and assessment — complimentary
We examine the gap, the neighbouring teeth and your gum health, and take X-rays to check there's enough bone to hold an implant. You'll be told honestly whether you're suitable, and what the alternatives are if you aren't.
Placing the implant
A single appointment under local anaesthetic. The titanium post is placed into the bone and the gum closed over or around it. Most people find this considerably easier than they expected — it is usually more comfortable than having a tooth taken out.
Healing — the long part
Over the following months the bone grows tightly onto the implant surface, which is what gives it its strength. This part can't be hurried, and it's why implant treatment is measured in months rather than weeks. A temporary tooth covers the gap meanwhile.
Fitting your new tooth
Once the implant is solid, impressions are taken and a custom porcelain crown is made to match the shade and shape of your own teeth, then fixed onto the post.
Keeping it healthy
Implants don't decay, but the gum and bone around them can become infected if plaque builds up — so regular check-ups and hygiene visits matter more with an implant, not less.
Am I suitable for an implant?
Most people are, but not everyone, and the assessment is what settles it. These are the things that affect it:
- Enough bone to hold the implant. Where bone has shrunk after a long-standing gap, it can sometimes be rebuilt, or a Teeth-in-a-Day approach used instead.
- Healthy gums. Active gum disease has to be treated first — an implant placed into infected gum is an implant at risk.
- Smoking, which meaningfully increases the chance of an implant failing. It doesn't rule you out, but you deserve to know it before you spend the money.
- Some medical conditions and medicines, including uncontrolled diabetes and certain bone medications. Bring your full medical history to the consultation.
- Grinding your teeth, which loads an implant heavily. It's manageable, usually with a night guard, but it needs to be planned for.
Implants have a high success rate, but no dental treatment succeeds every time and nobody honest will promise you otherwise. What we can do is assess you properly, tell you what your own risk factors are, and not place one where it isn't likely to work.

How long do dental implants last?
Implants are designed as a long-term replacement, and with good cleaning and regular hygiene visits they commonly last many years. The crown attached to the implant may need replacing before the implant itself does. Nobody can promise a specific number of years — how long it lasts depends largely on how the gum and bone around it are looked after.
Does getting a dental implant hurt?
The procedure is done under local anaesthetic, so you shouldn't feel it. Most patients say it was easier than having a tooth removed. Some soreness and swelling for a few days afterwards is normal and manageable with ordinary painkillers. Sedation is available if you're anxious.
How long does implant treatment take from start to finish?
Usually several months, because the bone needs time to heal onto the implant before the final tooth is fitted. You aren't left with a visible gap during that time — a temporary tooth covers it. Teeth-in-a-Day is the exception, where a fixed bridge goes on the same day the implants are placed.
Can I have an implant if I've lost bone in my jaw?
Often, yes. Bone can sometimes be rebuilt before an implant is placed, and Teeth-in-a-Day can work in situations where bone has already been lost. It's exactly what the X-rays at your consultation are for.
Can implants secure a loose denture?
Yes, and it's one of the most life-changing things we do. An implant-retained denture clips onto several implants instead of resting on the gum, so it doesn't lift or rock when you eat and talk, and you get considerably more bite force back.
Do implants decay?
The implant itself can't decay — it's titanium and porcelain. But the gum and bone around it can become inflamed and infected if plaque builds up, which can eventually loosen the implant. Brushing, cleaning between the teeth and regular hygiene visits are what prevent that.
Is it worth replacing a back tooth nobody can see?
Usually yes, for reasons that have nothing to do with appearance. Neighbouring teeth tilt into a gap, the opposing tooth drifts down, chewing shifts onto the other side, and the bone in that area gradually shrinks. Replacing it protects the teeth you still have.
